What is a Mental Health Care Plan?
A Mental Health Care Plan (MHCP) is a structured clinical document prepared by a General Practitioner that identifies your mental health concerns, goals for treatment, and referral pathway to a mental health specialist. It enables access to subsidised psychological sessions through mental health services, making support more affordable and accessible.
The plan outlines your symptoms, required support, and the number of therapy sessions you may receive each year. Most plans unlock up to 10 Medicare-subsidised sessions with a psychologist, social worker, or counsellor, helping you take practical steps towards recovery.

How the Review & Renewal Process Works
Your Mental Health Care Plan is valid for 12 months and includes scheduled reviews to ensure your treatment remains appropriate and effective.
Initial Review
(after 1–2 therapy sessions) Your GP checks your progress, makes adjustments, and confirms any further referrals.
Ongoing Review
(3 months after the initial MHCP appointment) You and your GP discuss your outcomes and whether additional support is required.
Renewal
(after 10 sessions or at 12 months): If you still meet clinical criteria, your plan can be renewed, allowing further Medicare-subsidised sessions.

How to Get a Mental Health Care Plan with 4Uniwell Health
We provide mental health services through secure telehealth appointments, supporting people experiencing stress, anxiety, low mood, emotional overwhelm, or other concerns.
1. Book Initial Consultation Appointment
Schedule your first MHCP appointment for AU$110 to discuss your mental health concerns and eligibility for a care plan.
2. GP Assessment
Discuss what you’ve been experiencing, such as feeling overwhelmed, sad, anxious, unmotivated, or mentally exhausted. Your GP may ask some questions and complete a brief psychosocial assessment.
3. Receive Your Plan
If you’re eligible, your GP will prepare a mental health care plan document and help refer you to a mental health specialist (e.g. psychologist, counsellor). You will receive a copy of the plan to take to your therapy sessions.
4. Review Appointment
Begin your sessions under the mental health arrangement, usually via 6 initial sessions, with the option to extend to 10.
Eligibility & Access
You may be eligible for a mental health plan if you:
Hold a valid Medicare card
Are experiencing mental health concerns impacting daily life, study, work, sleep, relationships, or wellbeing
Are not currently under a different plan for the same issue (e.g., a Care Plan for another condition)
Agree to a mental health assessment with a GP
People aged 16 years and above are generally suitable for telehealth or in-clinic mental health care plan appointments. Teenagers, university students, FIFO workers, full-time employees, and carers are highly encouraged to seek support early.
What Conditions Qualify for a Mental Health Care Plan?
A Mental Health Care Plan is available to people experiencing a range of emotional, psychological, or behavioural concerns that impact daily life, work, study, or overall wellbeing. During your first telehealth consult, one of our clinicians will assess your symptoms, discuss how they are affecting you, and determine whether a Mental Health Care Plan is clinically appropriate.
